As a healthcare-related service provider, Miora Health is required by law to:

  • Maintain the privacy and security of your protected health information (PHI)

  • Notify you if a breach of your PHI occurs
  • Follow the terms of this Notice

2. Information We Collect

When you use our services, we may collect personal and health information such as:

  • Health-related information you voluntarily provide through questionnaires
  • Test kit information and test results

This information is necessary to provide services, process orders, communicate with you, and deliver your results.

  • To provide testing services (e.g, shipping kits, processing samples, returning results)
  • To communicate with you about your order, test status, or support inquiries
  • For payment processing and order fulfillment

We do not

  • sell your information.
  • share your health data with third parties unless you explicitly authorize us to do so or it is required by law.

4. Your Relationship With Partner Clinics

If you choose to connect your test results with a clinic or provider through our platform:

  • You must enter a clinic-specific code in our portal to respond to a clinic’s invitation
  • Doing so grants Miora Health permission to share your personal and health information, including questionnaire responses and test results, with that clinic

This connection can be removed upon request, but any previously shared data may remain in the clinic’s own records


5. Your Rights Under HIPAA

You have the right to:

  • Request a copy of your health information
  • Ask for corrections if you believe your information is incorrect
  • Request limits on how your information is used or shared
  • Request confidential communications
  • Receive a copy of this Privacy Notice
  • File a complaint if you believe your privacy rights have been violated

6. Your Rights Under HIPAA

We use appropriate safeguards to protect your data, including encryption, secure storage, and limited access controls. Only authorized personnel have access to your information for purposes of providing and managing your services.


7. Permission for Use and Disclosure of Information

By accessing the Miora Health platform, you acknowledge that you have read this agreement, had the opportunity to ask questions, and obtain satisfactory explanations, and that you authorize the use and disclosure of protected health information as described here.


8. Duration of Terms

These Terms of Use remain in effect unless and until they are revised or terminated by Miora Health or the patient. Miora Health reserves the right to update these terms at any time, and patients will be notified of any changes. A patient may revoke access to their information at any time.
